*Responsibilities:*
* Provide direct and individualized nursing care to residents based on their unique needs and care plans.
Administer medications and treatments as prescribed, ensuring accuracy and adherence to established protocols.
* Monitor and assess residents' health status, promptly identifying and responding to changes in condition.
Perform routine health checks and collaborate with healthcare professionals for comprehensive care.
* Work closely with interdisciplinary teams, including physicians, therapists, and care aides, to ensure coordinated and comprehensive resident care.
Communicate effectively with residents, families, and team members to provide updates and address concerns.
* Maintain accurate and up-to-date resident records, documenting assessments, interventions, and outcomes.
Ensure compliance with regulatory standards and assist in audits as needed.
* Assist residents with activities such as bathing, dressing, grooming, and mobility to promote independence and well-being.
* Provide health education to residents and their families, promoting preventive care and overall wellness.
*Qualifications:*
* Current and valid LPN license in NY.
* 1+ years of experience in a similar healthcare setting, preferably in assisted living or long-term care.
* BLS certification required.
Job Type: Contract
